Enrollment Information

When you enroll your student in school, we collect information to allow us to best serve your student in our schools. The information we collect includes previous educational records, student addresses, phone numbers to contact you, and emergency contacts. Completing the forms online can save you a lot of paperwork when going to the school.

Your child will be enrolled when you fill out the form and then bring the necessary items listed to the school office.

Items to Bring

If your child has never been enrolled in a WV school please bring

  • Birth Certificate (must be certified from the state)

  • Immunization Record

  • Social Security Card
